We insist that every staff member observe patient confidentiality and respect your right to privacy about your medical records and experience with us. We will not disclose any medical information revealed by a patient or discovered by a care provider in connection with the treatment of a patient. We will only share data outside of our patient care team for legal purposes or as clinically necessary. While you may be asked to provide medical information in completing patient registration forms found on this web site or in the office, we can assure you this information will be treated with the same care we treat our medical records. Any information collected will be used to help us provide individualized services that meet your medical needs. We will not share your individual identity or personal contact information with anyone for commercial purposes.

We collect information necessary for processing governmental, commercial insurance, and health programs. This information is used to identify you and to determine your eligibility. It is also used to decide if the services and supplies you receive are covered by these programs and to insure that proper payment is made. Appropriate disclosures of information may be made to other providers of services, carriers, intermediaries, medical review boards, health plans, and other organizations or federal agencies for the effective processing of your insurance claim for payment. Other than stated in our privacy policy, we will not share your information unless it is medically indicated and necessary for your health and well-being.

Whenever necessary, parents (and/or patients over age 18) will be asked to sign a medical record release form authorizing Lakeview Pediatrics, LLC to release your medical records to a designated third-party.
