Location

We are located on Belmont Avenue in the Lakeview neighborhood, just east of the intersection of Ashland, Lincoln, and Belmont. The address is 1525 West Belmont Avenue, Suite 103. Street parking is available on Belmont Avenue and the neighboring streets. If you turn south on Greenview and right into the first lot, there are several spots reserved for patients. The lot behind that is a city lot, where you can park for 25 cents per 30 minutes.
