Insurance
As a courtesy, we will bill your insurance company. However, you are responsible for all delinquent balances past 60 days. Any balances that have not been paid by the insurance company or you after 120 days may be sent to a collection agency. If you have any questions regarding your bill, please call our billing company, KLO, at 630-789-2550.

For the most part, we do not accept any HMOs, POS, EPO plans, except where specified. We do not accept public aid, Medicare, Medicaid, or Kid Care. Please be aware to contact our office with any possible insurance change for confirmation. Plan participation status frequently changes. Please call if you have a provider that is not listed.
